Phoenix, Feb 24. 2022 (GLOBE NEWSWIRE) — Health Current, a Contexture organization, Arizona’s health information exchange (HIE), today announced the launch of AzHDR.org, a robust online resource for accompany the new Arizona Health Guidelines Registry (AzHDR program).
The new website, designed for vendors, professionals who support end-of-life (EOL) decision-making, and consumers, simplifies logging into the registry and makes it easier to find advance directive information.
AzHDR.org offers a wealth of advance care planning resources, including how to start difficult conversations about end-of-life care, access to advance directive forms to document healthcare preferences, and instructions on how to register wishes in the registry. The site offers a range of articles, blogs, videos, tools, marketing materials and more.
Health Current became AzHDR’s new headquarters after 2019 legislation authorized the registry to transition from the Secretary of State’s office to Arizona’s HIE to improve healthcare providers’ access to advance directives.
Launched in fall 2021, the AzHDR helps honor patient preferences for end-of-life care by providing seamless access across the continuum of care to advance directive documents, including:
Living wills, power of attorney forms for health care and mental health care, pre-hospital medical care directives (often referred to as do-not-resuscitate or DNR orders).
The online system provides a trusted and secure place to store and make available advance directives for Arizonans so that end-of-life care is guided by their wishes. The secure and confidential registry provides peace of mind for registrants and provides easy access to participating healthcare providers and other professionals, ensuring that documented wishes are honored.
